住房公积金各储户：
为规范住房公积金的提取行为，进一步明确因偿还购房贷款提取住房公积金的审批工作，根据国务院《住房公积金管理条例》的相关规定和七届17次市政府常务会议的有关精神，经研究，现就因偿还购房贷款提取住房公积金所需提供的资料补充通知如下：
职工因偿还购房贷款提取住房公积金，需提供下列资料原件：
一、申请人身份证。
二、个人住房资金存折。
三、与银行签订的购房贷款合同。
四、贷款所购住房的房地产权证。
五、偿还银行贷款的凭证（申请当月有还款记录的“供楼”存折）。
委托他人代办的，属夫妻关系的提供结婚证原件，其他关系的提供委托人签名的书面委托书和代办人身份证原件。
本通知从2008年1月2日起执行。
